According to Michael Van Kesteren, the design was inspired by medical equipment to reflect the Rani's scientific nature, drawing particular inspiration from the design of a syringe, and in the look of white enamel. The red and brown pattern is directly inspired by Kate O'Mara's costume from Time and the Rani, which was based on photos of the costume that were sent to Van Kesteren by the archive team.
The purple crystal is inspired by the flower of Deadly Nightshade, because "it's beautiful and deadly".
