Could this be the model?

The proportions look right. It doesn't have a white phone panel, but everything else seems to match.
It was built by Matt Irvine in 1975 for 'The Pyramids Of Mars'.

I noticed in the Tales of The TARDIS version of Pyramids of Mars they reused a shot of the Johnathan Sellers Box from Shada and then funnily enough leave the later shot of the TARDIS shaking unchanged.

Looking at those production photos, I had a few thoughts:
First, the miniature work, particularly when the photos are in black & white remind me a lot of Hartnell establishing shots--particularly "Keys of Marinus", Secondly, when was the cultural shift? I'm going to say mid-to-late '90s. It's wild to see someone just casually holding a lit cigarette while working. I guess maybe they could've been using cigarette smoke to create atmosphere for the model shot, but I suspect no, the guy's just smoking while working.
