Early Hartnell:
Pretty much intact.

Late Hartnell:
Pieces and levers are gone.
Time rotor flag upside down.
Broken pieces on console

Early Troughton:
Grills were lost
Interior Walls were cut down, Photo walls were not.

Late Troughton:
Left Radiation Meter have been smashed
The right one was installed incorrectly since The Mind Robber
One of the flags are gone and slightly faulty rotor spinning
Green gels on the lights in the rotors

Ambassadors:
Bits replaced
Meters are the same from last time
Spinning rotor missing
Green gels still present

Inferno:
Circuit board lines
Radiation Panel rebuilt
Pieces and bits added on
Spinning rotor put back in incorrectly and is nonfunctional now
Green gels still present
Time rotor case crooked when in 'Powered-up' position

Decommissioned after Inferno

Alright, I'm going to be REAL specific. This list contains just Season 1 stuff.
The Pilot Episide
The first EVER variation of the console. It was pretty bland, dull etc. Made up of Domed lights and small, clear levers.
An Unearthly Child
The first canonical version of the console. It was updated, they would add grip knobs, more levers. They would be given color since clear stuff doesn't really show well on camera.
Edge Of Destruction
The first canon update to the console. Nothing much. All they did was add a fast return.
The Aztecs
A temporary change - on panel 2 the lone lever has a finger grip knob placed over it
Reign Of Terror
One of the square reflectors on the time column has been rotated.

I've got a list of sll the sixties stuff somewhere.

Not exhaustive, but quite a lot of stuff.
